    Layla Dobos Newbie

I need assistance in getting my Grandmother's DD-214 form. This is needed for a medical emergency / continuation of her benefits. Her name is Carmen Novia Bowman and her date of birth is 7/17/35. She was honorably discharged in / around 1956. Her name at the time of service was (per her birth certificate) was Carmen Rodriquez Novoa.

          Dear Ms. Dobos,

Thank you for posting your request on History Hub!

We suggest that you request a copy of her DD-214 from the National Personnel Records Center at St. Louis.  Please complete a GSA Standard Form 180 and fax it to 314-801-0764.  Provide all of her used names. Women’s records were filed by the surname at the time of last contact.  Also include her social security number, service number (if known), as well as date and place of birth. Indicate that the purpose for this request is for an urgent medical emergency.

You also may submit your request online through eVetRecs.  Indicate in the comments section that the request is for an urgent medical emergency. Please note that eVetRecs will be offline Thursday, January 7, 2021 from 8:00 p.m. until 11:59 p.m. CST. See eVetRecs Help for instructions. For more information about NPRC’s online system and to check the status of your request, please visit Request Military Service Records

Due to the COVID-19 pandemic, the NPRC is closed except for emergencies. Currently, NPRC will continue servicing requests ONLY associated with medical treatments, burials, and homeless veterans seeking admittance to a homeless shelter. If your request is urgent, please see Emergency Requests and Deadlines. Please refrain from submitting non-emergency requests such as replacement medals, administrative corrections, or records research until NPRC returns to pre-COVID staffing levels. Please check archives.gov/veterans for updates to the NPRC operating hours and status. We apologize for any inconvenience.

We hope this information is helpful.

          Sincerely,

          Holly Rivet
