We suggest that you request a copy of her DD-214 from the National Personnel Records Center at St. Louis. Please complete a GSA Standard Form 180 and fax it to 314-801-0764. Provide all of her used names. Women’s records were filed by the surname at the time of last contact. Also include her social security number, service number (if known), as well as date and place of birth. Indicate that the purpose for this request is for an urgent medical emergency.
You also may submit your request online through eVetRecs. Indicate in the comments section that the request is for an urgent medical emergency.
