What happened to my SF-50 Request?

    Mason Kirkland Newbie

Hello, I've submitted a request on 31 March to obtain my SF-50 Separation Form from the Federal Government in April 2019, and I've not received a copy yet. There's many jobs I'm getting rejected/disqualified from because I don't have a copy of that form. This is out of my control, and I would like for someone to help assist me in obtaining that ASAP.

          Dear Mr. Kirkland,

Thank you for posting your request on History Hub!

We suggest contacting the National Personnel Records Center, Annex to check the status your SF-50 request.  Their hotline number is (618) 935-3000, but you may also FAX to 618-935-3014.

Due to the Coronavirus public health emergency, the Civilian Personnel Records Center is working at reduced staff levels and only working high priority cases.  We thank you for your patience and look forward to resuming normal operations when the public health emergency has ended.

We hope this information has been helpful!

          Sincerely,

          Holly Rivet

            Dear Ms. Bowers,

This is an update to our previous response.

The Civilian Personnel Records Center closed on August 19, 2020 due to unsafe local public health metrics.  CPR will remain closed until local public health conditions improve. During the closure, CPR will maintain minimal staffing levels to support emergency reference requests only.  Since you are requesting the SF-50 for employment purposes and it is needed immediately, please resubmit your request.  Write "urgent" on the outside of the envelope to expedite the process.

We hope this update is helpful.

            Sincerely,

            Holly Rivet
