We recommend that you consult the third page of the SF-180 form. Please use the listing to determine which organization you will mail it to depending on your branch of service, date of final discharge, and rank. Please check for both your personnel record as well as your medical record. Often they are held at the same location, but this may not be the case since you served more recently.
