Thank you for your question submitted to History Hub! A NA Form 13164 is sent in response to a Freedom of Information Act (FOIA) request. It is included with what information is releasable according to the FOIA regulations. If you are requesting your own active service records, a FOIA is not required for access. However, it is anyone’s right to submit a request under FOIA even for those records that are releasable to you as the veteran. If you would like to request your own personnel record, you may do so online through this site with eVetRecs. This site also provides a direct link to a SF 180 which you may fill out and send in through physical mail.
In order to submit your request under FOIA, you need only to state “that you are requesting this information under the Freedom of Information Act (FOIA).” There is no specific form required when using the FOIA to request your personnel records.

If you wish to inquire into the availability of your unit records, please provide your list of units with specific dates to the National Archives at College Park, Textual Reference Branch via email@example.com. Thanks for your reply.....that was very helpful.
I have requested my records via eVetRecords, however, they force me to make 5 separate requests and I thus assume I will get 5 separate answers.
I will try the FOIA route and hopefully, I will receive ONE document (NA Form 13164) that lists all of my service history for the four branches and the NJ National Guard.
Any advice on how to make the specific FOIA request; what details should I provide?

Please include as much information about yourself as possible -- SSN, any Service Related numbers, dates of service in each organization listed, etc.
