In order to request an Official Military Personnel File, which will contain a DD214 as a part of his military service record, please complete a Standard Form 180 and mail it to:
National Personnel Records Center
1 Archives Dr.
St. Louis, MO 63138
Official Military Personnel Files are open to the public if the veteran was completely separated from service 62 years from today's date or prior. Thus, if your veteran left service on November 14, 1954 or prior then you receive copies of the record for a fee. You may find more information and the Standard Form 180 at: https://www.archives.gov/veterans/military-service-records
Thank you,

Re: copy of his DD-214, which is his military service record. Hoe kan ik aan deze komen? Where to get the digtal file?
Willem Anthony Apr 26, 2017 10:55 AM (in response to Cara Moore Lebonick)Hello Cara Moore,
Many thanks for the answer. I do not need the whole record. As you can seen on this image he is registered on 01-01-1946 as died on on 25-10-46. A bit confusing suposing he could have died in USA on an earlier date. So if he had served in the military, do I have to use the Standard Form 180 too? It is not determined yet if he had served.
I have downpoaded the form.

3 people found this helpfulYes, you would still need to fill out the SF-180 to verify if he served and when he left the service.
