We searched the National Archives Catalog and located the series Oath Ceremony Lists and Petitions for Name Change, 11/27/1991 - 8/30/1996 for the U.S. District Court for the Southern District of New York in the Records of District Courts of the United States (Record Group 21) that may include the document you are seeking. These records are arranged chronologically by date of ceremony, and the Petitions for Name Change are filed with the list of applicants sworn in at the oath ceremony. These records have not been digitized. Please contact the National Archives at New York (RE-NY) at newyork.archives@nara.gov for access to these records.
