I found this record on my family I'm not exactly sure what it is, can someone tell me what it is?

Ok thank you I will look into those, the next day I found another document and it was filled out completely not just names evena section that said American names that's where the Bentons names were but didn't give their birth names. Then yesterday I found a card (very yellow) that had numbers, with another set I believe it was document numbers and other numbers 2 Benton, husband and wife, listed on the top of this card it says  "Cherokee Nation", I believe the Tribe. I'm going to pull the last 2 documents back up, since they have much more information and write document numbers down so when I look into the site's you gave I'll be able to give them
