Information on my cousin, Capt Calvin T Ballard (Serv #O-434031) award of the DFC in WW2

Question

Calvin was in a B24 over Munch that was hit and crashed on July 21, 1944. He was KIA and Is buried at the Netherlands American Cemetery.
