How can I see samples of WWII-era DA Form 458. This is the Charging Document prior to a court-martial.

A copy of WG ADO Form 115 appears in Technical Manual TM 12-255 Administrative Procedures dated November 1, 1942 pages 199-202. You may be able to find a copy of this publication by searching online or you can view a copy of this publication at the National Archives in College Park, MD.
