    Individual medical reports of veterans who left the Army (before October 16, 1992), Air Force (before May 1994), Navy (before January 31, 1994), or Marine Corps (before May 1994) are in the custody of NARA's National Personnel Records Center (NPRC) in St. Louis.  Please complete a GSA Standard Form 180 and mail it to the National Personnel Records Center, (Military Personnel Records), 1 Archives Drive, St. Louis, MO  63138-1002.

     

    Due to the COVID-19 pandemic, NPRC will continue servicing requests ONLY associated with medical treatments, burials, homeless veterans seeking admittance to a homeless shelter, and those involving the VA Home Loan program. If your request is urgent, please see Emergency Requests and Deadlines. Please refrain from submitting non-emergency requests such as replacement medals, administrative corrections, or records research until NPRC returns to pre-COVID staffing levels.  Please check archives.gov/veterans for updates to the NPRC operating hours and status. We apologize for this inconvenience and appreciate your understanding and patience.

    For medical records of veterans who left the Army after 2013 should contact the AMEDD Records Processing Center, 3370 Nacogdoches Road, Suite 116, San Antonio, TX 78217.

     

    For medical records of veterans who left the Air Force after 2013 or enlisted National Guard not on active duty should contact the AF STR Processing Center, ATTN: Release of Information, 3370 Nacogdoches Road, Suite 116, San Antonio, TX 78217.

     

    For medical records of veterans who left the Navy or the Marine Corps after 2013 should contact the Navy Medicine Records Activity (NMRA), BUMED Detachment St. Louis, 4300 Goodfellow Boulevard, Building 103, St. Louis, MO 63120.

  • When you say "military spouse," was the spouse in the military, or are you referring to the spouse (or other family member) of someone who was in the military, because the disposition of medical records are different for family members than for service members.

    If it is for a family member, and you can provide the year the care was provided, and if it was for inpatient (they were admitted to the hospital) or outpatient (a visit to a clinic) I can ask the people who specialize in that sort of thing, as I work at the Defense Health Agency and can inquire of the patient record experts.

    Please don't provide any more details than that, due to patient confidentiality rules.
