Seeking information Henry Gilbert Jimenez US Army

Need to obtain Military records for Henry who is applying for Birth Certificate. His certificate has him listed with a different name. He is hoping military records will have the correct birth certificate so he can prove identity.

It depends when Henry was in the Army what agency has his Official Military Personnel File (OMPF). If he left the Army prior to October 2002, his OMPF is in the custody of NARA's National Personnel Records Center in St. Louis. Please have Henry complete a GSA Standard Form 180 and mail it to NARA's National Personnel Records Center, (Military Personnel Records), 1 Archives Drive, St. Louis, MO 63138-1002. Or he may apply online at https://www.archives.gov/veterans/military-service-records/.

After September 2002, his OMPF is in the custody of the U. S. Army Human Resources Command, ATTN: AHRC-PDR-VIB, 1600 Spearhead Division Avenue, Department 420, Fort Knox, KY 40122-5402.
