    Do blank questions on a form need to be transcribed?

      When transcribing a form, should all questions be transcribed or only those that are not blank.

          Thank for your question.  We suggest the following when you encounter a form that is blank:

           

          1.  Transcribe the text of the form, leaving the response areas blank or add [blank]

          or

          2. Add the following note to the top of the page: [Blank form not transcribed]

              If I might ask another question.

              In reading about transcribing forms on the training page, there seems to be little concern about formatting, most pertinent is getting the information in digital form. I was wondering if I should run Form question and answers horizontally to compress space, or if each question should have it's own line, i.e.:

               

              FORM QUESTION #1 : Answer  FORM QUESTION #2 Answer FORM QUESTION #3: Answer

               

              or

              FORM QUESTION #1: Answer

              FORM QUESTION #2: Answer

              FORM QUESTION #3: Answer

               

              Could you tell me which would be preferred?
