We searched the National Archives Catalog and located 23 series of the 199th Infantry Brigade (Light) during the 1960s; Provost Marshal Serious Incidents Reports, 1967; and the General Records, 1965 - 1973 of the Judge Advocate in the Records of the U.S. Forces in Southeast Asia (Record Group 472) that may include the report you seek. The information about his death may be contained in his Individual Deceased Personnel File (IDPF).
