If you are filling out the paper form and mailing it in, include the sections you want under the "Other" section.
If you want to do this online, you can do so by using the NARA site, eVetRecs. On this site you can request whatever sections you may want from your personnel file. This information goes in the "Comments" box on the "Documents Requested" tab. Once you finish the form, you can print it out, sign and date it, and mail it to
