The National Archives has various blank forms available, including census, immigration, and military draft forms. Here is the link.
The book by William Dollarhide titled "The Census Book, Facts, Schedules & Worksheets for the U.S. Federal Censuses" copyright 2019... ISBN: 978-1-62859-265-8 has an example of the form p. 208, and a list of types of questions asked p. 109. Perhaps you can get the book on interlibrary loan?
